How we scored it
Scores follow the published rubric in our provider comparison methodology.
Medical Disclaimer
We're not doctors, and this review is not medical advice. GLP-1 medications are prescription-only and require evaluation by a licensed healthcare provider. What works for one person may not work for another, and only a clinician who knows your history can determine whether a medication is appropriate for you. See our full medical disclaimer.
What Is MDLIVE?
MDLIVE is structurally different from almost everything else in this comparison, and that difference is the whole review. It is not a direct-to-consumer weight-loss subscription. It is a large virtual care company — urgent care, primary care, mental health, dermatology — owned by Evernorth, the health services arm of Cigna, and weight management is delivered inside its virtual primary care service.
That means access usually comes through your employer or health plan rather than through a credit card. MDLIVE states that if you have its virtual primary care as part of your health plan benefits and meet the medical requirements, you are eligible, and that "the copay for visits and the cost for prescriptions depend on your health plan benefits".
Clinically it is conservative in a way the cash-pay market is not. It prescribes FDA-approved anti-obesity medications — Wegovy, Saxenda and Contrave are named — at physician discretion. There is no compounded semaglutide or tirzepatide, so none of the regulatory questions that dominate the rest of this comparison apply. Care is delivered by a physician who takes a full health history, with digital health coaching, integrated lab work, mental health referrals and prescriptions routed to a local or mail-order pharmacy.
The trade-off is that you cannot price it in advance, and it is not open to everyone. MDLIVE excludes anti-obesity medication prescribing in Florida, Louisiana, Mississippi and New Jersey. If your employer does not offer it, this is not a service you can simply sign up for the way you can with the rest of this list.
How the Program Works
- Consultation process
- A virtual primary care visit with an MDLIVE physician who reviews your health history and discusses your goals, with same-day appointments subject to availability. This is a full primary care relationship rather than a weight-specific intake form.
- Eligibility
- You generally need MDLIVE virtual primary care as part of your health plan benefits, and must meet the medical requirements. Prescribing of anti-obesity medication excludes Florida, Louisiana, Mississippi and New Jersey.
- Provider involvement
- Board-certified physicians deliver care within a virtual primary care model, so the clinician sees your wider health picture rather than weight alone. Digital health coaching and mental health referrals are available alongside.
- Lab requirements
- Integrated lab work is part of the virtual primary care service, ordered clinically rather than as a fixed protocol.
- Pharmacy fulfillment
- Prescriptions are sent to a local pharmacy or mail order and billed through your pharmacy benefit. MDLIVE does not dispense, and there is no compounding involved.
- Shipping frequency
- Not applicable — fulfilment is through your own pharmacy.
- Support services
- Virtual primary care, digital health coaching, integrated lab work, mental health referrals, and access to MDLIVE's wider urgent care, dermatology and behavioural health services on the same account.
MDLIVE Pricing
- Membership fee
- None in the consumer sense. Access typically comes through an employer or health plan.
- Medication fee
- Billed through your pharmacy benefit rather than by MDLIVE. Because the medications are FDA-approved, manufacturer copay assistance may apply where you are commercially insured.
- Cancellation policy
- Not applicable in the subscription sense. There is no recurring consumer membership to cancel; access follows your health plan.
- Refund policy
- Governed by your health plan and MDLIVE's standard visit terms rather than by a consumer refund policy. We found no published cash-pay refund policy for weight management.
- Insurance
- The entire basis of the service, and its main advantage. MDLIVE is built to bill your plan, and because it prescribes FDA-approved medication, insurance can genuinely apply to the drug as well as the visit.
- HSA / FSA
- Visit copays and prescriptions are commonly HSA/FSA-eligible. Confirm with your plan administrator.
Pricing details worth knowing
- MDLIVE publishes no cash-pay price for weight management, because the model assumes plan coverage.
- If your plan covers a GLP-1, this is among the cheapest routes in this comparison — a copay rather than $150–$400 a month.
- If your plan does not cover GLP-1s, MDLIVE cannot fix that, and a cash-pay provider or a manufacturer direct pharmacy will be cheaper.
- FDA-approved medication only means no compounding questions — and also no sub-$200 compounded pricing.
Prices change. Every figure above was taken from MDLIVE's published pages or the named sources at the bottom of this review, as of August 8, 2026. Telehealth pricing in this market changes frequently, and promotional rates expire. Confirm your actual cost at checkout before enrolling.
Pros and Cons
Pros
- FDA-approved medication only — no compounding questions at all
- Delivered inside virtual primary care, so a physician sees your whole health picture
- Owned by Evernorth, Cigna's health services arm, rather than a recently launched startup
- Integrated lab work, digital health coaching and mental health referrals included in the model
- If your plan covers the medication, a copay is far cheaper than any cash-pay programme here
- Same-day appointments subject to availability
- Prescriptions go to your own pharmacy, so you can shop the price
Cons
- No published cash-pay price, so you cannot compare it in advance
- Access usually depends on your employer or health plan offering it
- Anti-obesity medication prescribing excludes Florida, Louisiana, Mississippi and New Jersey
- No compounded option, so it cannot compete on cash price
- Weight management is part of primary care rather than a structured weight programme with a defined curriculum
- If your plan excludes GLP-1s, MDLIVE offers no route around that
Who MDLIVE May Be Best For
Anyone whose employer or health plan already includes MDLIVE virtual primary care — this is the cheapest route in that case
People who want FDA-approved medication and a primary care relationship rather than a single-purpose weight service
Patients who would rather have weight treated alongside their other conditions than in isolation
Limitations
Check two things before anything else: whether your plan includes MDLIVE virtual primary care, and whether your pharmacy benefit covers GLP-1 medication. The second matters more, and MDLIVE cannot change it.
If you live in Florida, Louisiana, Mississippi or New Jersey, anti-obesity medication prescribing is excluded.
If you are paying cash, this is not your service. NovoCare Pharmacy sells Wegovy direct at $149–$199/month and compounded providers start under $200.
We could not verify a published consumer refund policy or a cash-pay price for weight management, because the model is benefit-based.

Frequently Asked Questions
How much does MDLIVE weight management cost?
There is no published cash price, because the service is designed around health plan coverage. MDLIVE states that the copay for visits and the cost for prescriptions depend on your health plan benefits. If your plan covers both the visit and a GLP-1, this is likely the cheapest option in this entire comparison — a copay rather than a $150 to $400 monthly programme. If your plan does not cover GLP-1 medication, MDLIVE has no way around that, and a cash-pay provider will serve you better.
Does MDLIVE prescribe Wegovy or Zepbound?
MDLIVE names Wegovy, Saxenda and Contrave among the anti-obesity medications its physicians may prescribe at their discretion. All are FDA-approved, so there are no compounding questions and insurance can genuinely apply. Note that prescribing of anti-obesity medication is excluded in Florida, Louisiana, Mississippi and New Jersey.
Can I sign up for MDLIVE directly?
For weight management, generally not in the way you can with a cash-pay telehealth company. MDLIVE states that eligibility follows from having its virtual primary care as part of your health plan benefits and meeting the medical requirements. Check with your employer or insurer first — if it is included, you may already have access without realising.
Does MDLIVE offer compounded semaglutide?
No. MDLIVE prescribes FDA-approved medications only. That means it cannot compete with the sub-$200 monthly pricing of compounded providers, and it also means none of the regulatory questions surrounding compounded GLP-1s — the FDA warning letters, the 503A and 503B rules, the unnamed pharmacies — apply here at all.
Who owns MDLIVE?
MDLIVE is an Evernorth company. Evernorth is the health services arm of Cigna. That is worth knowing for two reasons: it is an established operator rather than a recent entrant, and its incentives sit with the payer side of the system, which is part of why the model is built around plan coverage rather than cash pay.
Who should not use MDLIVE?
Anyone paying cash, who will find it both unpriceable in advance and more expensive than direct alternatives. Anyone whose plan does not include it. Residents of Florida, Louisiana, Mississippi and New Jersey, for anti-obesity medication specifically. Separately, GLP-1 medications are not appropriate if you have a personal or family history of medullary thyroid carcinoma or MEN 2, if you are pregnant or trying to conceive, or if you have had pancreatitis.
The Verdict
MDLIVE is a large virtual care provider owned by Evernorth, Cigna's health services arm. Weight management runs through its virtual primary care service rather than a standalone programme: a physician can prescribe FDA-approved anti-obesity medication including Wegovy, Saxenda and Contrave, and what you pay is your health plan's copay. There is no cash-pay price and no compounded medication.

How We Researched This Review
We compiled this review on August 8, 2026 from MDLIVE's own published pages plus independent reporting. We have not signed up for this service ourselves, and we say so rather than implying firsthand testing. Where MDLIVE does not publish a detail — refund terms, lab requirements, pharmacy partners — we've said that plainly instead of guessing.
