Supplements are not medicines
Dietary supplements are not medicines. In the United States they are regulated as food rather than drugs, which means the FDA does not review them for safety or effectiveness before they go on sale, and manufacturers cannot legally claim a supplement treats, cures, or prevents any disease. Nothing on this page is a substitute for prescription treatment, and no supplement described here has been shown to produce the weight loss seen with semaglutide or tirzepatide. If you are considering a supplement — particularly alongside prescription medication — talk to a doctor or pharmacist first, because interactions are real and some ingredients are not appropriate for everyone.
The Checklist
1. Find the Supplement Facts panel — not the marketing copy
The panel is the regulated part. Everything else on the box is advertising. Read the panel first, then read the marketing and notice the gap between them.
2. Look for per-ingredient amounts
Every active ingredient should have a number next to it. If several ingredients are grouped under a “proprietary blend” with one combined weight, you cannot evaluate the product against research and neither can anyone else.
A practical heuristic: if the dose were impressive, it would be on the front of the box. Hidden amounts are usually hidden for a reason.
3. Compare the amount to the research dose
This is the step most people skip, and it is the one that decides whether a product can work.

If a capsule contains berberine plus five other actives, arithmetic tells you it is not delivering 900 mg of anything.
4. For probiotics, check for CFU or AFU counts
Organism count is the dose. A probiotic that does not publish one is asking you to buy on trust. Also look for strain-level identification — genus and species at minimum, ideally the strain designation, because probiotic effects are strain-specific.
5. Look for third-party testing
NSF, USP, or a named independent laboratory program. This verifies identity, potency, and absence of specified contaminants. It does not verify efficacy. Given documented potency variability in some ingredients — berberine among them — it matters.
6. Read the “other ingredients” line
Capsule material (hypromellose is vegetarian, gelatin is not), fillers, flow agents, and any allergen statement. If you have significant allergies, the absence of an allergen statement is itself information.
7. Check the warnings
A thorough contraindication list is a good sign, not a bad one. It suggests the company thought about who should not take the product. A label with no warnings at all on a product containing live bacteria or botanical extracts is doing you a disservice.
Phrases That Mean Less Than They Appear To
- "FDA registered facility" — the site is on a list. Says nothing about the product.
- "Clinically studied ingredients" — a study exists on an ingredient somewhere, possibly at a dose this product does not contain.
- "Doctor formulated" — unregulated phrase, no standard behind it.
- "Natural" — legally near-meaningless, and not a synonym for safe. Berberine is natural and interacts with statins.
- "Proprietary blend" — you are not being told the doses.
- "Supports" — the legally required hedge. It is not a claim of effect.
The two-question test
How much of the active ingredient is in a serving? If the label will not say, put it down.
Is that amount within the range used in published research? If not, the research does not apply to what you are holding, however impressive the ingredient sounds.
A Worked Example: Running the Checklist on a Real Label
A checklist is easier to agree with than to use, so here is a real product read through it. We have picked one that is genuinely mixed rather than obviously bad, because those are the ones that need the checklist.
BetterBrand's BetterLungs is a capsule marketed to “clear your airways and support healthy lungs and sinuses”, aimed at people dealing with excess mucus from smoking, environmental exposure or seasonal irritants. Two capsules contain vitamin D 25mcg, NAC 400mg, mullein leaf 300mg, pine bark extract 150mg, elderberry 100mg, American ginseng 50mg and reishi mushroom 37.5mg. It is $34.95 for a 30-day bottle, $31.45 a bottle for three, or $27.95 a bottle on subscription, and carries a 365-day guarantee.
What the label does well
- Every amount is disclosed. No proprietary blend. You can see exactly how much of each ingredient you are getting, which is the first thing on the checklist and the thing most products in this category fail.
- The FDA disclaimer is unusually complete. It states the claims have not been evaluated by the FDA or any agency, and that the products are not designed for diagnosing, treating, curing or preventing disease or altering any bodily structure/function. That last clause goes further than the standard boilerplate.
- The guarantee is long and specific. 365 days is generous by the standards of this market.
What the checklist then reveals
Now apply the harder questions — which ingredient is doing the work, at what dose, and in whom?
The hero ingredient has the weakest evidence. Mullein is what the brand builds its educational content around, and there is no good human clinical evidence that mullein leaf clears lungs or improves respiratory disease outcomes. The European herbal monograph does not establish clinical efficacy for it. What exists is in vitro and animal work — including a 2020 Journal of Ethnopharmacology study finding bronchodilatory activity in extracts — which is a reason to keep studying it, not a reason to expect it to do something for you.
The ingredient with real evidence is under-dosed for the population studied. NAC is the interesting one. It has a genuine mucolytic evidence base: meta-analyses published in 2024 found NAC reduced exacerbation rates in COPD (incidence rate ratio 0.76, 95% CI 0.59–0.99) and in chronic bronchitis or pre-COPD (IRR 0.81, 95% CI 0.69–0.95), and the PANTHEON trial found 600mg twice daily prevented exacerbations in moderate-to-severe COPD. Earlier meta-analyses found no benefit, so it is contested rather than settled.
But look at the numbers side by side. The studied dose is 1,200mg a day, in people with diagnosed COPD or chronic bronchitis. The label here is 400mg per serving, in people who do not have those diagnoses. That is a third of the dose, in a different population, for a different purpose. None of which makes it useless — it makes it something the research does not speak to.
A regulatory quirk worth knowing about NAC
NAC sits in an unusual position. It was approved as a drug on 14 September 1963, and in March 2022 the FDA concluded that this excludes it from the definition of a dietary supplement under the exclusionary clause. In guidance published in the Federal Register in August 2022, the agency said it intends to exercise enforcement discretion while it considers rulemaking that would permit NAC in supplements.
So NAC on a supplement shelf is there on regulatory forbearance rather than on the normal footing, pending a decision that has not been made. That is not a safety warning. It is exactly the kind of thing a label will never tell you and that changes how you read it.
The overall verdict from the checklist is a fair one: a transparently labelled product, with an honest disclaimer and a long guarantee, whose marketing leads with its least-evidenced ingredient and whose best-evidenced ingredient sits below the dose used in the trials, in people the trials did not study. That is a much more useful thing to know than “works” or “scam”.
And the thing no supplement does
If you are considering a product like this because you vape or smoke, be clear with yourself about what is on offer. No supplement reverses inhalation injury, and none has been shown to undo the damage that stopping would prevent. Anything in this category is at best an adjunct to stopping, never an alternative to it.

Frequently Asked Questions
If a label discloses every dose, is the product good?
Disclosure is necessary, not sufficient — it is what lets you ask the next question rather than being the answer to it. Once you can see the amounts, check two more things: which ingredient the marketing leads with versus which one has the evidence, and whether the dose matches what was actually studied and in whom. A fully transparent label can still show you a hero ingredient with no human trials behind it, and an evidence-backed ingredient at a third of the trial dose in a population the trials never included.
Why do some supplements contain ingredients that are also drugs?
Because the line is drawn by history rather than by pharmacology. Under the FD&C Act's exclusionary clause, a substance approved as a drug before being marketed as a supplement is excluded from the supplement definition. N-acetylcysteine is the well-known example: approved as a drug in 1963, and in March 2022 the FDA concluded it is excluded from the dietary supplement definition — then said in guidance that August that it would exercise enforcement discretion while considering rulemaking to permit its use. So it remains on shelves under a stated forbearance rather than a settled permission, which a label will never tell you.
What is a proprietary blend and why does it matter?
A proprietary blend lists several ingredients with only a combined total weight, so you cannot tell how much of each you are getting. It is legal, and it is the most common way to make a formula look impressive without committing to a dose. If a product uses one for its key active ingredients, you cannot compare it against any research.
What do NSF and USP marks mean?
They indicate independent third-party verification that the product contains what the label says and is free of specified contaminants. They do not verify that the product works. In a category the FDA does not pre-screen, they are one of the few available quality signals.
Does "FDA registered facility" mean the FDA approved it?
No. Facility registration means the manufacturing site is on an FDA list. It says nothing about the product, its ingredients, or whether it works. It is one of the most commonly misread phrases in supplement marketing.
What claims are red flags?
Anything about treating, curing, or preventing a disease — those are illegal for supplements. Specific weight-loss figures. Before-and-after photos. "Nature\u2019s [drug name]" framing. And any claim of a guaranteed result, which no honest supplement can offer.
